I think you cannot do it by your own. You need to find a travel agent to do it for you as international travelers are request to travel as a group tour in Tibet.

Passenger Health Registration Card Lhasa Railway
Hy. So, I've been thinking on going to Lhasa from Guangzhou by train, but recently I have read about a Passenger Health Registration Card, where I have to sign to say that I am healthy due to the lack of oxigen. If I have asma, would that affect me? Thank You.

I did not read about a health registration card have to be offered to enter Tibet. The travel permit to enter Tibet is a must. But due to the high altitude, it would be better to ask a doctor first to see if your health can fit the condition there.

You can take the flight or go by rail by your own, but the pernit is needed for boarding. You can not visit there by individual, you have to do it with a tourist group. The permit need to apply from the Tibet Tourism Bureau instead of the embassy.

Dear Goh. You must refer to Xining, right? Well, the train takes around 21 hours.

It is not allowed to travel to that area on your own. According to the regulation, you have to join a tour through a Chinese travel agency. They would help you have Travel Permit, with which you shall be allowed to enter that region.

You can book the ticket through internet, or phone calls. Or you can buy it in advance in any railway station. If it is not convenient for you, I suggest you a better option. I had booked through travelchinaguide several times, and they save much trouble for me. You can turn to them, too. I remember their contact info is bookingtravelchinaguide.com.
