I am an education consultant sending students to China for studies. There is one Physical Examination Form which the student is needed to submit after thorough check-up by a doctor of a public hospital. Now the form which the schools send us are 2 pages but stragely a couple of reputed hospitals in Mumbai have 3 page forms. This form is not available for download, neither can we get blank forms from the hospital. While applying sometimes the embassy asks for the third page..which we do not have.
How can we get this 3-page form if that is necessary?
Oct. 9,2009 01:57
Mr.Peter replied:
You can pick up the Physical Examination Form from the Visa office of the embassy.
Oct. 9,2009 03:51
Mr.Sanjay(India) replied:
Thanks for the reply Mr. Peter,
But the embassy is far away at one end of the vast Mumbai city. Moreover it is open for a very short period everyday.
Oct. 9,2009 19:53
Mr.Lee replied:
Then you can download the Physical Examination form from this website.

Oct. 20,2009 06:11
Mr.Sanjay Sharma(India) replied:
Thanks Mr. Lee.
But as I said in my email, this is again only 2 page form. Which I know and have it downloaded. Some big hospitals in Mumbai have a 3 page form. although the 3rd page is repetition but still they insist on it. And no link is available to download it.
Thanks anyway.
Sanjay

I live in Bangalore, and need F visa for a fair in Shanghai.Kindly let me know as to where to send my application and other documents for Visa. Is it a must to hire a agent, or can the application be sent thru post. I ahve visited Tibet Autonomus Region- China on a group Visa, do I need to send details of that along with the application. Appreciate your answer to pathalam@gmail.com
Oct. 9,2009 19:58
Mr.Hilary replied:
You can either apply for the china visa from the China embassy in Mumbai, India or from the Consulate-general of China in Salt Lake City, Kolkata. You can submit your application in person, or if you can't go to the embassy or consulate in person, you can entrust a visa or travel agent to process it for you on behalf, but the China embassy and consulate don't accept visa application by post. You have to present your former travel records to China along with the other documents.
