Day 1 Beijing - Luoyang
Today you will take the high speed train for about 4 hours to Luoyang by yourselves. We will book the high speed train ticket of Beijing/Luoyang for you in advance and mail it to your Beijing hotel. Your private local tour guide in Luoyang will pick you up at the railway station and transfer you to the downtown hotel. The remainder of the day is free for you to have a good rest or explore the city.

Day 2 Luoyang
Longmen Grottoes

The private tour today includes the Chariot and Horse Pits, Luoyang Museum and Longmen Grottoes. The Chariot and Horse Pits are the accessory pits to a tomb of Eastern Zhou Dynasty. A visit to the pits will give you a general knowledge of the unique funeral custom between 770 BC and 256 BC. Not far from the Chariot and Horse Pits, you will come to Luoyang Museum. The large storage of historical items and all kinds of exhibitions will show you the glorious culture and history of the ancient city. The Longmen Grottoes is one of the four greatest grottoes in China. The Buddhist statues with various gestures and expressions are densely dotted here, reflecting the splendid Buddhist culture between the 4th century and the 12th century.

Day 3 Departure from Luoyang
Today you are going to visit the famous world heritage, Shaolin Temple along with the Pagoda Forest. Initially built in 495 during the Northern Wei Dynasty, Shaolin Temple is noted as the birthplace of Shaolin Martial Art. The temple, located at the foot of Songshan Mountain among dense forest, is also the cradle of Chinese Zen Buddhism. The Pagoda Forest is one of the most important attractions in Shaolin Temple. Covering an area of over 20,000 square meters (about 5 acres), the Pagoda Forest consists of 231 ancient tomb pagodas. After that, our guide will transfer you to the airport and see you off there for your onward flight.

I think you mean Yun Tai Shan?
If so, yes, you can have a day trip there. You are suggested to take a bus to Jiaozuo first, and then take a taxi or private to the mountain.

What bus can I take from Luoyang Longmen Bus Station to Shaolin Temple?
How often does it run? What is the bus number? How much will it cost? Is it the same bus/frequency/cost back?

No bus is available from Longmen Station.
You need to take Bus 33 to Luoyang Station first.
After that, walk across the road to the long-distance bus station and take a regular bus to the temple. The bus costs CNY19 and the same fare for back trip.
