Notice: The Polish consulate in Shanghai accepts visa application between 09:00-12:00, on Tuesday, Thursday. Chinese passport holders need pay RMB 240 for visa application. Generally, it takes 7 or 10 working days to process visa application. If visa application is denied, the consul officials wouldn't reply the reasons for denial and the applicants only can submit application after one year again.

 Please contact China Embassy in Poland if Polishes will go to China.

This is not the official website of Embassy & Consulates of Poland in China, so the required documents can't be found if the official website doesn't provide the information, I think you'd better go to the consulate of Poland in Shanghai in person to get such information.

Later, I search relative regulations. I find news released by the Ministry of Foreign Affaires of China. According to the bilateral agreements, Chinese Diplomatic/Service (Official) holders can be exempt from visa to the six member countries of Schengen Convention: Poland, Hungary, Slovakia, Slovenia, Lithuania and Malta. However, they should come to the six countries by direct flight. If they need transit other Schengen countries in order to go to the above six countries, they need gain the visa of destination country before departure.
