 Guangzhou to Haikou Train
Z385: depart at 04:57 and arrive at 15:15 with travel time of 10h18m;
Z201: depart at 16:02 and arrive at 03:40 the next morning with travel time of 11h38m.

 Haikou to Guangzhou Train
Z386: depart at 11:50 and arrive at 22:22 with travel time of 10h32m.
Z202: depart at 20:45 and arrive at 08:20 the next morning with travel time of 11h35m.
 

Transfer at Haikou Railway Station for more options

The above Guangzhou – Sanya train schedule may not satisfy every passenger’s needs, when a transfer at Haikou Railway Station is recommended. By this way, high speed trains are available for the ride between Haikou and Sanya. But a transfer between Haikou Railway Station and Haikou East Railway Station is needed. Passengers can take bus line 35 to travel between directly which takes about 2 hours and CNY 5.

1. Overnight sleeper trains should be the top choices for passengers who need to take a long distance travel. From Guangzhou to Sanya, Z201 which departs at night and arrives the next morning is recommended. It saves a lot of time as well as some money. Train no. Z202 should be the primary choice for passengers who want to travel from Sanya to Guangzhou.
2. Since foods provided on the train are simple and a little expensive, passengers should better prepare their favorite food for the long travel. Portable charger is also a necessity in a long distance travel. Most importantly, daily necessities like tooth brush, tooth paste and towel also need to be packed in the suitcase.
3. The travel time onboard may be quite boring, so one can choose to look out to appreciate the different sceneries from one place to another.

Change trains in Haikou
Is this Haikou station which Z111 stops at the same one where I can take a high speed train to Sanya?

There are around 10 high speed rides a day from this station to Sanya, but most of the high speed rides go from Haikou East. If you decide to make a transfer in Haikou, you'd better plan plenty time between. Z111 runs a long distance before reach Guangzhou. It may delays.

is there a rail line between sanya and the mainland
does the train go on to some sort of ferry or how does it work, get off the train get on a ferry then get a seperate train on hainan island

Yes, there is rail service to the mainland. The train will be aprt and board a ferry. After landed, it will keep running to the destination. Passengers will keep stay inside the carriage during the whole process.

I may need to make a transfer in Maoming and Haikou to get a ticket to Sanya.
However, I have one concern. D7487 from Guangzhou will reach Maoming at 10:22pm, but Z111 from Maoming to Haikou only departs at 2am the next day. So am I able to stay inside the station all the time? I will have to give up if I have to stay outside more than 3 hours at midnight time.

I think it should be no problem. Maoming Station must be on duty for 24 hours and stations should allow passengers who are about to take the train in 3 hours.
